Hello, I’m
Dr.Jyothi Patri

“My journey in medicine began with a deep desire to understand and heal, evolving into a lifelong mission to lead, teach, and transform the way we practice medicine through empathy, innovation, and purpose.”

About Me

I am Dr. Jyothi Patri, a double board-certified physician in Family and Lifestyle Medicine. I am an educator, leader, consultant for Graduate Medical Education and primary care health systems, and a patient advocate.

“I believe in meeting patients where they are, understanding their stories, and guiding them towards sustainable wellness.”
My practice is grounded in connection and continuity. Integrating evidence-based medicine with the principles of Lifestyle Medicine allows me to help patients make informed, lasting changes that support both physical and emotional health. For me, sustainability extends beyond environmental stewardship – it means cultivating mindful, compassionate, and enduring practices for patients, clinicians, and healthcare systems alike.

“My passion for education has been the foundation of my leadership journey.”
Immersed in academic medicine, I have developed programs and curricula that promote excellence, wellness, and professional growth amongst trainees. This fusion of clinical expertise, patient advocacy, and commitment to lifelong learning has shaped me into a well-rounded physician leader – one who strives to inspire the next generation to approach medicine not merely as a career, but as a calling rooted in humanity and sustainable care.

My Clinical Journey

From rural clinics to academic hospitals, my clinical path has been grounded in the belief that every patient’s story matters. My experiences as a frontline physician and as a patient advocate shaped my philosophy that medicine is both science and humanity. These encounters honed my diagnostic acumen and strengthened my resolve to empower patients with knowledge, compassion, and continuity of care.

Family and Lifestyle Medicine

As a Family and Lifestyle Medicine Physician, I integrate evidence-based medicine with holistic wellness. My approach emphasizes prevention, mindful living, and personalized care plans that promote sustainable health. I believe healing begins when patients become partners in their wellness journey.

Graduate Medical Education

My commitment to medical education extends beyond teaching, it is about shaping future leaders. I have designed curricula, spearheaded residency programs, and mentored physicians to excel in clinical reasoning, leadership, and resilience. My initiatives have strengthened GME programs through innovative teaching models, quality improvement projects, and well-being frameworks that inspire growth and balance.

Leadership and Advocacy

“Leadership, for me, is service with purpose. “
I have led interdisciplinary teams, launched wellness initiatives, and championed advocacy efforts that elevate both patient and physician voices. To me, being a physician leader means standing at the intersection of care delivery and system transformation – bridging the gap between evidence, empathy, and equity.

Recognition and Impact

“Meaningful impact begins with intentional work and a shared vision for better healthcare.”
Driven by a commitment to education, innovation, and physician growth, I have been honored through regional, national, and international presentations, publications, and awards. From developing resident-led teaching models to enhancing graduate medical education through curriculum innovation and wellness design, each recognition serves as a reminder that impact is measured not by achievement alone, but by the lasting influence we create – in shaping future physicians and fostering a culture of excellence, compassion, and continuous growth.

“Exploring art, culture, and new experiences brings us depth, clarity, and purpose.”
Whether hiking in nature, traveling to gain new perspectives, or creating art with loved ones, these pursuits allow me to reconnect with what truly matters. They strengthen empathy and resilience – qualities that enrich how I lead, teach, and care. Living mindfully reminds me that growth extends beyond the clinic, and I hope to inspire future physicians to live with intention – cultivating harmony between work, relationships, and the world we share.

What my Colleagues Say About me

I'm Dr. Kida Thompson, a Family Medicine physician based in Las Vegas, Nevada. I first met Dr. Jyothi Patri when she was my senior resident during my internship at the University of Illinois College of Medicine in Rockford. Dr. Patri embodies two remarkable qualities - she is tenacious and resourceful. These traits mean there is very little she cannot accomplish once she sets her mind to it. She approaches every task with enthusiasm and thoroughness - a level of dedication and completeness I have rarely seen in my career.

Hi, my name is Dr. Ellis! Dr. Jyothi Patri and I were colleagues and partners at the Transitional Care Clinic in Fresno, California. It’s been wonderful having Dr. Patri as a colleague. As a family physician myself, coming from a hospital medicine background, it’s been refreshing to see how she approaches things in a primary care setting, she is very thorough and deeply compassionate toward her patients. She’s also highly committed to her community and to the field of family medicine.

Hi, I’m Dr. Diana Howard. I first met Dr. Jyothi Patri at California Academy of Family Physicians (CAFP), where she was serving as the President of the first Fresno-Kings-Madera Chapter. During that time, I learned the value of focus and leadership from her. She showed me that residents truly have a voice to advocate for themselves and for their patients. Dr. Patri has been an incredible source of inspiration for me. Seeing how a minority woman from the Central Valley can make such a profound impact on her peers and her future patients has been truly empowering.

My name is Emely, and I work at the Traditional Care Clinic with Dr. Jyothi Patri. I worked as her Medical Assistant, and I can honestly say that it’s been an honor to work with her. What stands out the most to me is her dedication and compassion. Being in the room with her has shown me how deeply she cares for her patients. She takes the time to sit with them face-to-face, listens with empathy, and makes sure they feel truly heard and cared for. Working with her has been inspiring, she’s not just a great doctor, but also an incredible role model for everyone around her.

Hi, my name is Dr. Jerry Neria, and I’m the current Medical Director of Living Water Clinic with Clinica Aguaviva Health Systems in Central California. She served as the Program Director of the Adventist Health Family Medicine Residency in Hanford, while I was leading the Tulare Program. I have witnessed Dr. Patri’s determination, grit, and unwavering commitment to excellence. I saw her transform a program that once struggled with uncertainty into one filled with vision, passion, and residents who thrived under her leadership. Even today, many of them continue to say how much they wish she were still their Program Director.

I’m Dr. Trisha Bautista — a Family Medicine physician and Addiction Medicine specialist. I currently serve as the Medical Director for Substance Use Services at Rikers Island and NYC Health + Hospitals Bellevue. I first met Dr. Patri when she became my incoming Program Director and anyone who knows her knows that she is an absolute force. She came in with remarkable leadership, energy, and resilience, helping to heal not just our residency program but also the broader Central Valley community, which was still recovering from the challenges of the COVID era.
